• SIN Number
• Birthday Information
• Information for any children in your full-time care
• If filing as a couple, both people have to be present.
• Ontario Works recipients are required to provide T5007 from Niigaanin
Please note: This will be operated as a walk-in clinic and will operate on a first come, first served basis. Please expect a wait time, and we request and encourage those attending to be kind.
