Dashaye Eshkakogan


Aanii, my name is Dashaye Eshkakogan Igrew up in Sagamok Anishnawbek. At very young age, I always knew I wanted tobecome a nurse. I was compassionate, empathetic and I enjoyed helping people. Iwanted to be able to provide care to our elders in our community.  

I graduated from Cambrian College in2021 from the practical nursing program and obtained my license. I have had many practicum placements working in the hospital. My background includes working at the Awo Taan Healing Lodge Society in Alberta as a Research Assistant. I also had the opportunity to work as the Home and Community Care Coordinator at the Elders Eagle Lodge.

I am passionate about my role as a nurse, and I am looking forward to working with my community as the Indigenous Transition Facilitator.  

My role in this Position is to coordinate and provide proactive continuity of care to strengthen our linkages with other services.

  • To navigate with health services.
  • Assisting clients with discharge from hospital to home.
  • Assisting with referrals prior to client leaving hospital. Such as, NELHIN, Physiotherapy, Occupational therapy, Home and community care supports and mental health.
  • Refers clients to alternate informal support networks to determine available supports.
  • Initiating early client identification for promoting client recovery from substance abuse and/or mental health disorders.
  • Monitoring client treatment progress and reassessing clients routinely
